ich vermute du hast L-Thyroxin seit Längerem eingenommen und Apalutamid kam neu hinzu? Falls noch nicht erfolgt unbedingt einen Mindestabstand von 30 Minuten zwischen den Einnahmen einhalten.
In diesem Fall scheint Apalutamid die Aufnahme oder Effektivität des L-Thyroxins aber herabzusetzen. In einer der Zulassungsstudien wurde hier ein Einfluss auf die Schilddrüsenachse vorbeschrieben:
„Hypothyroidism was reported for 8.1% of patients in the apalutamide group versus 2.0% in the placebo group. Only grade 1 and grade 2 events were observed. Hypothyroidism led to treatment discontinuation in 1 patient, dose reduction in a second patient, and dose interruption in no patients in the apalutamide group; treatment discontinuation, dose reduction, or dose interruption due to hypothyroidism were not observed in the placebo group. Hypothyroidism in patients treated with apalutamide worsened in patients already receiving thyroid replacement therapy. Generally, elevations in thyroid-stimulating hormone occurred early in treatment; median time to first thyroid-stimulating hormone elevation in the apalutamide group was 113 days. Hypothyroidism was diagnosed with regular thyroid-stimulating hormone monitoring, and was

Die Autoren empfehlen also eine Dosissteigerung und das halte ich in der Situation nach Rücksprache mit deinem behandelnden Arzt auch für sinnvoll. Sofern du nicht schon jetzt Beschwerden einer Überfunktion hast würde ich auf 275µg oder gleich 300µg pro Tag steigern und in 6-8 Wochen wieder kontrollieren.
